Contact Address

In this module, the user can add or edit one address at a time for the selected contact, indicate if the address is primary, and enter a date when an address is no longer valid.

 

If the record is linked to a Global Institution, then some fields may be editable and other fields must be edited in the source module where the record was created. The source modules include Global Contacts, Global Institutions, or My Settings. You can create a copy of a linked address by clicking the Copy button. The linked record will continue to be available in the list of address records and the new unlinked record can be modified. To inactivate a record, you must be in the source module.

 

When an Address Is No Longer Valid

  1. In the Thru Date field, enter the date that the address was no longer valid or the date that you learned that the address is not valid.

  2. Click Save, which returns you to the previous Address tab and makes the address Not Current if the date is prior to today's date.

 

To Make an Address Primary

  1. To add another address and make it primary, on the previous Address tab, click the Add New Address button or to make a current address primary, click next to the address that is current, which will return you to this module.

  2. Place a check in the Primary box on the Type row.

  3. Click Save, which returns you to the previous Address tab.

Adding a Communication Record from Global Contacts, Site Staff or Global Institution

Description

Institution

Using the dropdown list, select the institutions that are housed in the Global Institutions database.  If the contact type is Site, then the list of institutions will be restricted to Research Facility. If the contact type is Other, then the list of institutions will be restricted to Other, IRB or SMO.  If the contact type is CRO, then the list of institutions will be restricted to CRO.  If the contact type is Project, then the list of institutions will be restricted to System.  If the contact type is Sponsor, then the list of institutions will be restricted to Sponsor.  This field will be read only when editing an Address record that has been inherited from the Global Contact's Institution.

Department

Enter the department that the contact works at for the Institution on the following field. The user can enter up to 200 characters.

Address

Enter the Institution's street address or PO Box. The user can enter up to 200 characters. This field will be read only when editing an Address record that has been inherited from the Global Contact's Institution.

City

Enter the city of the Institution. The user can enter up to 50 characters. This field will be read only when editing an Address record that has been inherited from the Global Contact's Institution.

State

Select from a list of states/provinces/territories for the country selected in the Country field. This field will be hidden if no states/provinces/territories are defined for the country. The user can enter up to 50 characters for those without a list. The caption for this field is determined by the value of the State Caption field for the selected country from the Country Dialog on the System Tables page. This field will be read only when editing an Address record that has been inherited from the Global Contact's Institution.

Postal Code

The user can enter up to 10 characters.  field will be read only when editing an Address record that has been inherited from the Global Contact's Institution.

Country

Select from a list of records from the Country table in the System Tables. This field will contain the default country used in the configuration of the system. This field is required and is read only when editing an Address record that has been inherited from the Global Contact's Institution.

Type

Select the type of address from the following choices:

  • Business

  • Business2

  • Home

  • Main Office

  • Other

When inserting a new record, the  default value is Business. This field is required.

Comment

The user can enter up to 200 characters.

Date Range

These fields are used only for informational purposes and do not restrict usage within the system. If the current date is outside of this date range, the record will be listed in the 'Not Current' group of Address records (unless it is the Primary Address record).

From Date:

The From Date is required. It will default to the current date when adding a new record.

Thru Date:

The Thru Date is used to indicate when an address is no longer current or valid.
